Pediatric Care Management (CM) Program
The Pediatric Care Management program addresses the health care needs of members under 20 years of age. The role of the Pediatric Care Manager is to coordinate care, as well as educate and collaborate with the member's parents to facilitate care.
Children with Special Health Care Needs
NHP is committed to working with providers for Children with Special Health Care Needs. We promote the AAP’s “medical home” model of care and want to identify supportive and collaborative partnerships with you to improve the care and quality of life for these children.
Upon the recommendation of our family advisors, Neighborhood Health Plan hired a parent of a special needs child to be a resource for other parents of CSHCN. In her non-clinical role, our Parent Consultant provides information and support to families of Children with Special Health Care Needs. She offers emotional support and linkages to support groups and other parents of children with special needs as well as special education issues and resources.
