When a member joins New Neighborhood Health Plan, they receive a "welcome call." During the call, a Health Needs Assessment is performed where we gather information about the member's health and/or psychosocial needs.
If needed, members are referred to NHP's internal case management programs and/or external community-based programs and resources for assistance.
