At NHP, we recognize that sometimes the reason people don’t receive excellent medical care is not medical in nature.
That is why our Social Care Management Team engages with members in need to help them access community-based services and programs that help them live healthier, safer, more productive lives.
Our Social Care Managers bring great experience and passion as they work with members in need to determine the types of resources you and your family may be able to access. Some of these resources include:
- Income programs (public assistance)
- Housing services
- Food programs
- Utilities assistance (your gas, electric, or phone service)
- Services for people with disabilities
- Making appointments and finding transportation
