This information may be updated as we continue to expand the capabilities of our web site. Neighborhood Health Plan may also make changes to the products described within the web site.

Neighborhood Health Plan makes this site available to provide general information for educational purposes about our products and services, health-related issues, and sources for additional information. The health information on this site and the interactive responses provided in secured sections of this site should be used for educational purposes only. They are not intended as medical, healthcare, or other professional advice. Neighborhood Health Plan will attempt to keep the information on our web site current and accurate but we do not make any guarantees, and we disclaim any implied warranty or representation about its accuracy, relevance, timeliness, completeness or appropriateness for a particular purpose. Neighborhood Health Plan may change information on this web site without notice. Privacy, Security, and Confidentiality

Neighborhood Health Plan strives to protect the privacy of each member’s Protected Health Information (“PHI”) and other personally identifiable information. We use and disclose such information only according to our strict confidentiality policies and in accordance with applicable federal and state laws designed to protect a member’s privacy. Our Member Privacy Practices explains how we use information about our members and under what circumstances we may disclose that information.

Our web site provides you with the opportunity to voluntarily submit forms to request additional information about Neighborhood Health Plan. If you do so by e-mail, and not through a password-protected section of our web site, Neighborhood Health Plan cannot guarantee the confidentiality or security of the information that you send. E-mail sent over the Internet is not secure. Therefore, do not use these e-mail forms, or use e-mail in general, to provide us with PHI, other personally identifiable information, or other detailed medical information. Neighborhood Health Plan will not be liable for any breach of confidentiality resulting from the use of e-mail via the Internet.

At no time will Neighborhood Health Plan sell, rent, loan, trade or lease any information collected on our web site. However, please note that we may need to use this information within the Neighborhood Health Plan family of affiliate companies or with a third party vendor to perform administrative, technical, or other functions that help us provide service to you. We seek to hold third party vendors to the same privacy practices and protections as Neighborhood Health Plan.

Except as noted above, Neighborhood Health Plan will not release PHI, or other personally identifiable information to another party unless required to do so by government authorities, legal processes, or otherwise required by law.

Secured sections of the site may contain PHI or other personally identifiable information. To access these sections, users will set up a secure login and password so they can safely perform certain online transactions. Implementing this security requires Neighborhood Health Plan to link a visitor back to his or her identity. To do so, we must gather identifiable information about users including information about the user’s computer as listed below, and information related to the user, such as pages visited and information that was submitted or changed.

To protect against unauthorized access to the PHI, or other personally identifiable information that is accessible in the password-protected sections of our site, Neighborhood Health Plan has adopted stringent security standards. Data transmitted between your browser and the password-protected sections of this site is protected by 128-bit encryption. The information that we collect from users through the password-protected sections of the site is confidential and is available only to authorized users. It is protected from unauthorized access through multiple layers of security controls and monitoring.

Any information collected through the web site is not stored on the web site, but will go to back office systems in the same manner that information from paper forms, submissions and phone calls to customer care are currently handled. The information is used as defined above. As a standard practice, as described below, we collect general statistical information on the content viewed, which is used to help us provide better service to you.

For all visitors to our web site, some information is collected when you access the web site. This information is not personally identifiable, but provides us with aggregate "usage data". This information is used to measure the number of visitors to the different sections of our site and to help us make our site more useful to our visitors. This information is retained and periodically deleted from the site. The collected information, such as the IP address, geographic location, operating system, browser software, and internet service provider, is not used to solicit business or to provide addresses for direct marketing material.

Neighborhood Health Plan’s web site employs "cookies" to provide visitors with information that is specific to them. A "cookie" is a packet of information sent by a web site to a user’s computer, which is then sent back by the computer each time it accesses that web site. These cookies do not contain any personally identifiable information, and they cannot be accessed or “read” by any web site other than Neighborhood Health Plan’s web site.
