Health Needs Assessment and Screening

New Neighborhood Health Plan members receive a "Welcome Call." During the call, a Health Needs Assessment is performed, gathering information about members' health and/or psychosocial needs.

If needed, members are referred to NHP's internal case management programs and/or external community based programs/resources for assistance. The types of assistance may vary from procuring food, clothing, or housing assistance to helping members choose appropriate health care providers.
