Diabetes Disease Management
NHP's Diabetes Disease Management Program helps providers identify and direct care management resources (primary and secondary preventive care) to their diabetic patients. The program is aimed at improving preventive screening rates to the at-risk population.
A Diabetes Care Manager makes outreach phone calls to members who have diabetes based on a defect list, and collaborates with the member/family, PCP and other health care providers to implement a care management plan. The Diabetes Care Manager also provides member education regarding diabetes as well as educating members about their benefits. The Diabetes Care Manager collaborates and provides co-management of diabetic members with other NHP Care Managers.
