Identifying Members at Risk for Acute Hospitalization

NHP uses claims data to identify members who are risk for acute hospitalization within the next year.

Our care management staff work with the member, the member's family, and primary care provider, to develop patient-centered goals, encourage responsibility for health care, assist in the coordination of care and facilitate supportive relationships.

Members will be discharged from the care management program when all efforts to meet their goals are achieved.
