Neighborhood Health Plan has a team of Social Care Managers who have experience in community-based services and programs. These programs can help you take care of your basic needs - such as housing, food, and utilities - so you can focus on improving your health.

A Social Care Manager can help you determine the types of resources you and your family may be able to access. Some of these resources include:

  • Income programs (public assistance)
  • Housing services
  • Food programs
  • Utilities assistance (your gas, electric, or phone service)
  • Services for people with disabilities
  • Making appointments and finding transportation
