Social Care Management - Connecting to community resources
Neighborhood Health Plan has a team of Social Care Managers who have experience in community-based services and programs. These programs can help you take care of your basic needs - such as housing, food, and utilities - so you can focus on improving your health.
A Social Care Manager can help you determine the types of resources you and your family may be able to access. Some of these resources include:
- Income programs (public assistance)
- Housing services
- Food programs
- Utilities assistance (your gas, electric, or phone service)
- Services for people with disabilities
- Making appointments and finding transportation
Smart Neighbor is NHP's database of community programs, social services, and other public benefits and resources available to our members, including children's services, domestic violence programs, food pantries, and more.
