What Is Care Partnership?

For those NHP members who are identified as having or being at risk for serious health problems or hospitalization, a Care Manager may contact you to find out how you've been doing managing your medical needs. When the Care Manager calls you, he or she will explain how the program works, how it helps members by giving them special support for their medical needs, and how he or she can help make sure you are getting all the care and services you need.

How Does It Work?

When you choose to have a Care Manager help you, he or she works with you and your doctor to create a plan of care that is right for you. You, your Care Manager and doctor work together to keep you healthy, and with this extra help, you may be able to avoid future health problems and hospitalizations.
