"For You Two"

Neighborhood Health Plan's "For You Two" program provides care management services to pregnant members at high risk for adverse birth outcome. We are strongly committed to making these services available to women with medical or psychosocial conditions that render them vulnerable to preterm or low birth weight delivery. We will enroll your pregnant patient into the "For You Two" program when you fax us a completed Universal Obstetric Risk Assessment form at 617-772-5512. One of our nurses will communicate with you about any woman engaged in ongoing case management.

NHP's obstetric nurse care managers work closely with obstetric providers to maximize access to and the efficacy of prenatal care. If engaged in care management, the member will have access to mental health or substance abuse services, direct referral to NHP's smoking cessation counselor, and full coverage of breast pumps and postpartum home visits.

Alere Medical Inc.

In partnership with Alere Medical Inc. (Alere), NHP provides specialty management services to low birth weight, medically complex and high-risk infants and their families. Alere provides comprehensive care management to newborns being treated in our network hospitals. This collaborative, neonatologist-directed program addresses the needs of these infants from the day of admission to the NICU through any required transitional and home care services.

Alere care managers are RNs with substantial NICU experience. They interact with hospital medical and nursing staff, as well as parents, to ensure comprehensive care management of NHP-enrolled babies. In addition, NHP will be assigning a pediatric nurse practitioner to selected high-risk newborns upon hospital discharge. The nurse practitioner will coordinate the baby's ongoing care, provide home visits, and be available 24/7 for advice and triage.
