Neighborhood Health Plan Honored with National Case Management Award

Social Care Management Program Recognized as Successful Model


BOSTON, June 8, 2011 - Neighborhood Health Plan (NHP) won the Case In Point Platinum Award for its Social Care Management Program which exemplifies working across the care continuum to improve health care.

The Case In Point Platinum Awards are hosted by Case In Point Weekly, a national case management resource guide and aim to set the standard for programs that deliver sustained success across a variety of disciplines and settings in the overarching continuum of care coordination. Finalists in the social work category demonstrate an outstanding social work case management program that coordinates care as well as resource management throughout the care continuum.

"We are honored to be recognized for our innovative social care management program," said Deborah Enos, president and CEO of Neighborhood Health Plan. "Our health plan is unique in its holistic approach to ensuring that health care is delivered effectively to underserved populations and those with complex care needs."

NHP's Social Care Management team includes 15 social care managers who help the most fragile populations achieve the best health possible assisting in meeting 'basic' needs. The team helps hundreds of members every month access food, housing, income, clothing, fuel assistance, and help in getting to medical appointments.

"The team works collaboratively with NHP�s medical and behavioral health care managers to best serve our member population," said Nicole Devanna, Manager of Social Care Management for Neighborhood Health Plan. "The integrated approach helps ensure quality care for those who need it most."
