ID Card

All NHP members receive an identification card. You should always carry both your Neighborhood Health Plan card and your MassHealth card whenever you go to the doctor or the pharmacy.

Need a New ID Card?

If your ID card gets lost or stolen, you can replace it by calling the Customer Service Center at 800-462-5449 or 800-655-1761 (TTY).
