Reminder to Members: If your personal information changes....

When your personal information changes, please notify your employer and/or NHP. Failure to provide timely notification may impact your insurance coverage and may result in your being billed for health care services provided to you.

If you have changes to the following information, you must provide it immediately to your employer and/or health insurer:

  • Home Address/Phone Number
  • Marriage
  • Birth of a child
  • Adoption of a child
  • Legal guardianship of a child
  • Court ordered coverage decisions
  • Divorce
  • Remarriage of insured
  • Dependent turning 19 years old
  • Marriage of a dependent
  • Termination of a dependent's student status
  • Death of insured
  • Death of a covered spouse or dependent

Eventually, there may be personal financial responsibility associated with the lack of timely notification of this health coverage information to employers and health plans.

Notifying your employer and health plan of these events as soon as you know of them is one way you can help streamline the delivery of health care and therefore help avoid unnecessary costs to you, your employer and throughout the system.
